Reinstatement Information
The Board of Physical Therapy is self-supporting and receives no general fund tax monies. It is soley funded through licensing fees.
If you did not renew, your license or certificate expired March 31, 2009.
Cease working until your license is reinstated.
In order to reinstate your credential, send the following to the Board office:
1. A completed Reinstatement Application (PDF-45KB)
2. A certified check, cashier's check or money order made out to Kentucky Board of Physical Therapy for $150
3. Copies of your continued competency certificates completed between April 1, 2007 through March 31, 2009, including the Jurisprudence Exam certificate. (see 201 KAR 22:045)
PT: Thirty (30) contact hours
PTA: Twenty (20) contact hours
4. If you have not completed the Jurisprudence Exam for the biennium ending March 31, 2009, click https://secure.kentucky.gov/pt/jurisprudence/.

Complete this form and mail or fax to Board office for change of address (home or worksite). Name changes require a copy of the legal document authorizing the change. Your records will be changed in-house only. If you wish to purchase a new wallet card or wall certificate, send your payment as indicated below.
Wallet Cards - $10.00 - Send $10 payable to KY Board of Physical Therapy with your request to Board office.
Wall Certificates -$20.00 - Send $20 payable to KY Board of Physical Therapy with your request to Board office.

Verification Request (PDF-82KB) Do you need to verify your KY PT/PTA credential to another agency? Submit a signed request to the board office along with the fee of $15 for each verification.
