The Board of Physical Therapy is self-supporting and receives no general fund tax monies. It is solely funded through licensing fees.
Take the JE for 2011-2013 biennium...it is available right now!! This exam is required for new applicants, reinstatement of license, and continued competency.

Address Change - You may now change your home address or physical therapy worksite information online. Use this form to change information online. Name changes must be submitted to board office with a copy of the legal document authorizing the change. Your records will be changed in-house only. If you wish to purchase a new wallet card or wall certificate, send your payment as indicated below.
Wallet Cards - $15.00 - Send check for $15.00 payable to KY Board of Physical Therapy with your request to Board office.
Wall Certificates -$25.00 - Send check for $25.00 payable to KY Board of Physical Therapy with your request to Board office.
